Request Images

For your convenience you may request images by completing the form below. Faxed requests will be followed within 24 hours of request receipt (Monday through Friday, 7:30 a.m. to 5:00 p.m.). Please call our file room staff at (951) 658-9243 to verify images are available for pick prior to your arrival at Hemet Valley Imaging.

Instructions:

• Fax the completed form to (951) 765-5415
• Pick up requested materials at 1001 East Latham Avenue, Suite G, Hemet CA. 92543.
• Mailed materials require proof of identification faxed with this form.
• Allow 24 hours for processing.
• Materials will be held for 48 hours after requested pick up date, after such time, materials must be re-requested.
